Session Description
Description: The session aims to focus on some of the challenges to characterize treatment responses in oncology. Characterizing the treatment response in general and exposure-response in particular is essential for approval of new drugs. The relevance of this topic is highlighted by FDAs Project Optimus. Survival metrics in many recent oncology studies demonstrate clear signs of non-proportional hazards (NPH). This violates the proportional hazard assumption of the traditional log-rank and Cox proportional hazard tests. Although a wide range of parametric and non-parametric methods for NPH has been proposed, there is no consensus on the best approaches. Prof. Hooker and colleagues have been involved in a project sponsored by the European Medicines Agency to address this. Over the past two decades, antibody-drug conjugates (ADCs) have emerged as an important new class of drugs in oncology. Compared with traditional chemotherapy, ADCs combine the target specificity of antibodies with chemotherapeutic capabilities of cytotoxic drugs. The aim is to widen the therapeutic window for the cytotoxic drugs and be able to achieve higher on target concentrations with less of target toxicity. However, efficient dose optimization remains an essential challenge to fully benefit from the advantages with ADCs. Joint models for longitudinal biomarker response and survival metrics have been shown to be a valuable approach to accurately capture how time dependent patient conditions affect survival. More traditional sequential modeling approaches can be biased due to factors such as immortal time bias and shrinkage of empirical bayes estimates.

Session Description
Description: Dose selection is driven by exposure-response (E-R), combined with dose-response and dose-exposure analyses. However, confounding factors may yield artificially flat or over-exaggerated E-R relationships. This problem has been on full display in recent years for monoclonal antibody therapeutics (mAbs), especially in oncology, where clearance-response relationships have been mis-interpreted as E-R relationships within a single dose level in phase 2 development and beyond. The problem is exacerbated with time-varying clearance, which is observed for many mAbs and is coupled to improvement in disease status. It is important to recognize that E-R confounding is not limited to mAbs in oncology and may occur broadly with other diseases and therapeutic modalities. Despite a growing recognition of this problem, underlying mechanisms leading to E-R confounding are poorly understood, as are the markers that may help to detangle the role of exposure in driving the response. In addition, the guidance for exposure metrics/pharmacometric approaches that yield confidence in dosing decisions remains to be developed. Awareness of these issues is important during study design and data analysis to avoid misinterpretation of spurious E-R relationships and ultimately improve dosing recommendations. This symposium will highlight pitfalls encountered in E-R analyses, provide insight into current understanding of confounding mechanisms for both small and large molecules, provide industry, academia, and regulatory perspectives, share case studies, as well as current strategies to identify and minimize the confounding. During the panel discussion, a path forward to address the gaps and enhance the value of E-R analyses for dosing decisions will be explored.

Session Description
Description: Alzheimer's disease (AD), a progressive neurodegenerative disease leading to dementia, imposes a substantial burden on patients, caregivers, and the healthcare system. Among the major diseases, AD is also one of the least well understood. Due in part to the long pre-symptomatic phase of the disease, the causal relationships between key pathological features (amyloid plaques, tau tangles, inflammation, and neurodegeneration) are poorly understood, as is the relationship of these features with cognitive and functional decline. As a result, the history of AD research is littered with failed development programs. With the emergence of novel biomarkers (plasma, CSF, brain imaging PET markers) and associated models, clinical trials were able to better target efficacious doses, leading to the approval of anti-amyloid monoclonals that have demonstrated slowing of disease progression. Diverse pharmacometrics approaches (pharmacologic, empiric and mechanistic disease models) were critical to address Right Dose, Right Patient, Right Study, Right Time, Right Approach questions. In the future, emerging data will provide deeper insight into disease processes and their impact on cognition, leading to even more efficacious treatments. The role of pharmacometrics in the fight against AD is a compelling example of "Past as Prologue, Bridges to New Horizons". In this session, we introduce the historic role of pharmacometrics in designing and interpreting the clinical trials leading to the recent approvals. We show how pharmacometrics is well positioned to leverage the emerging data to enhance our understanding of the disease state and help optimize therapeutic strategies with the goal of eliminating AD.
